They stop stress eating, eating due to depression and anxiety, binge eating, and nighttime eating, too. And the medications usually keep on working indefinitely. When I mention using medications to help lose weight, you may have frightening thoughts of “diet pills” that cause medical problems or even addiction. You may remember the problems with “phen- fen” back in the mid- 9. You’re probably aware of the recent ban on ephedra. You may have had disappointing experiences with Xenical or Meridia. You may have lost weight with diet pills, only to regain when the pills were stopped. So naturally, you’re skeptical about taking medication to help lose weight. The good news is that we now have very safe medications to stop food cravings with virtually no side- effects in the vast majority of patients. My years of experience, and the many years of experience of my colleagues around the world attest to the safety of these medications. I keep up with the research in this area very carefully, and I am not aware of even one medical paper that has documented significant medical problems with these medications. Of course, there’s no need to lose weight if you’re going to regain it. I know that overweight isn’t like a sore throat that we treat and then forget; it’s like arthritis: we get it under control and then we work lifelong to keep it under good control. Fortunately, these medications can be safely used as- needed lifelong to help maintain the weight you lose. Once we have your food cravings stopped with medications, you can follow any type of weight- loss diet easily. I’m flexible and will work with your diet preference; however, the diet that I have seen the best results with (and the one I follow myself) is the low- carbohydrate diet. This diet produces the fastest weight loss, with no hunger and no medical problems. At least 4 medical studies in the past few years show that a low- carb diet results in faster weight loss, more weight loss, and improved cholesterols and triglycerides. Fears about ketones, osteoporosis or kidney problems on low- carb diets have been shown to be unwarranted. Low- carb diets work best, probably, because they lower your insulin level the best. Insulin causes body fat storage: the more insulin you make, the more fat you store. A low- carb diet causes the least increase in insulin levels. With all the new low- carb foods and restaurants today, it’s easy to follow a low- carb diet for the rest of your life. I’ve been on my low- carb diet since 1. Of course, exercise has a place in weight loss—but perhaps not what you’d expect. We’ve learned that the best use of exercise is to lower insulin. Remember, lowering insulin reduces stored body fat. The type of exercise that lowers insulin the best is strength training. By strength training we mean weight lifting, or using the weight machines at a gym, or using a Bow. Flex- type machine, or simply using the elastic band exercisers you can buy at Wal- Mart. The really good news is that strength training takes just 3. With that schedule you’ll add more muscle protein for a full 2 years, lowering your insulin and further reducing your fat storage. Combined with a low- carb nutrition program, strength training will help you lose the most excess body fat. Aerobic exercise (jogging, walking, running, swimming, treadmills, Nordic Tracks, stair- steppers, dance, etc.) are good for your heart and lungs; but they don’t do much to help you lose excess body fat (unless you run several miles per day). So switch to strength training twice a week for the most effective weight- loss exercise. My overweight treatment program starts with a private visit between you and me. We’ll review your weight history, your pattern of overeating, past dieting efforts and your general medical history. We’ll review your new low- carb nutrition plan. At the end of that visit we should be able to start you on the most appropriate medications to stop your food cravings within 4. Within one week of starting my program you’ll notice significant weight loss. Within 3 weeks your clothes will start feeling loose. When you come back to see me one month after the first visit, you’ll tell me that you’re amazed at how easy it is to follow this program and lose weight. You’ll tell me that food cravings are gone; that it’s easy to avoid the fattening foods that you could never resist; that you’re never hungry—and that the medications are working with no side- effects. You’ll feel wonderful that you’re on your way to achieving—and maintaining—your weight- loss goal. Within 6 weeks people will start asking you what you’re doing to lose so much weight. By then, any hip and knee pains will be much better; gastric reflux will be almost gone; you’ll be sleeping better and have more energy. By then, if you’re on medications for high blood pressure, reflux or diabetes we’ll probably start reducing them. Within the first 4 months, you’ll probably lose 1. Starting Weight. Typical weight loss in first 4 months. After the first 4 months, you can expect to lose about 5 pounds per month until you reach your goal weight. At that point the program doesn’t abandon you. We’ll switch to maintenance treatment to make sure that you don’t ever regain the weight you’ve lost. You and I will always meet at least twice a year to monitor your progress. And will meet immediately anytime you regain 5 pounds that you can’t get off within 4 weeks. We’ll use medications as- needed to keep your weight where you want it. You’ll notice that I have an entirely new approach to the treatment of overweight. I understand that it is a chronic disorder that will require lifelong monitoring. I don’t expect you to “be perfect” in following any diet. I know that life has ups and downs and that stresses or temptations occur that increase carb cravings for a short while. We stay with the long- term plan, make any short- term treatment adjustments needed and focus on success. My attitude is always positive; no scolding or blame ever. Many senior citizens are doing very well in my program. As long as they have no unstable heart disease, they can take the medications very safely and with good results. I treat many patients with diabetes in my program, too. They also are able to take the medications and lose weight well.
